Position Overview:

The Risk Adjustment Coder is responsible for coordinating implementation of programs designed to ensure all diagnoses are coded according to CMS and risk adjustment coding guidelines and conditions are properly supported by appropriate documentation in the participant chart.

Essential Job Functions:

Coding:
  • Review and assign accurate medical codes for diagnoses performed by providers and other qualified health care providers
  • Identify and communicate documentation deficiencies to providers to improve documentation for accurate risk adjustment coding
  • Understand the anatomy, pathophysiology, and medical terminology necessary to correctly code diagnoses
  • Collaborate with providers to implement measurable, actionably solutions that will result in improved accuracy for documentation and coding practices
  • Understand the audit process for the Hierarchical Condition Categories risk adjustment model
  • Maintain a clear understanding of guidelines, regulations, diagnosis coding, and risk adjustment as well as adhere to federal, state and local regulations
  • Educate providers and staff on coding regulations and changes as it relates to quality and risk adjustment to ensure compliance
  • Performs analysis and provides feedback to providers on a regularly scheduled basis
  • Facilitate communication with auditors and consultants on diagnosis and clinical documentation
  • Adhere to Bluestem policies and procedures
  • Review and capture diagnosis codes from prior medical records
  • Ensure hospital documentation supports diagnosis codes
  • Monitor and follow up on Encounter Data System (EDS) process


Qualifications and Experience:
  • 1-year experience working with the elderly
  • Excellent organization and attention to detail
  • Exceptional written and oral communication skills
  • Proficient with computers and Microsoft Office
  • Certification preferred (CCA Certified Coding Associate, CCS Certified Coding Specialist, CPC Certified Professional Coder, CRC Certified Risk Adjustment Coder)


Physical Requirements:

Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently. Requires walking or standing to a significant degree; or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls.
