Administers speech and language rehab procedures as ordered by the referring physician, provides services to patients of all ages, administers standardized and non-standardized assessments, obtains and interprets data necessary for treatment, implements and evaluates the patient's plan of care, works with family and health care professionals to meet the needs of the total patient. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Essential Responsibilities:

• Performs an assessment within patient's home or residential environment that is comprehensive, age appropriate and specific to patient need.
• Formulates a plan of care from the comprehensive assessment that includes a diagnosis and measurable goals and objectives.
• Maximizes the patient's abilities in a safe, effective and appropriate manner, prioritizing patient needs and utilizing available resources.
• Evaluates therapy to determine the effectiveness in obtaining goals and objectives.
• Complete reassessments in a timely manner in accordance with regulations by CMS.
• Insures functional outcomes and continuity of care through discharge planning.
• Assess patient/family preferences, goals/needs, and implements appropriate measures to meet these.
• Performs patient care and department documentation that is timely, accurate and concise.
• Demonstrates awareness and sensitivity to patient/family rights.
• Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
• Has reliable transportation and able to travel within a minimum of 50-mile radius of Hutchinson.

Minimum Qualifications:

• Education: Master's degree from an accredited college in Speech-Language Pathology

Experience:

• Minimum 1 year of experience in acute care, outpatient or home health setting.

Certifications/Registrations Required:

• Current licensure in Speech-Language Pathology through the State of Kansas
• BLS - BLS is required within 3 months of hire
• Kansas State Driver's License
• Auto Insurance equal to or greater than the Kansas Insurance Requirement
